REGISTRATION FEE
Individuals: $840. Save on group rates. Register and pay in a group and receive a discount. Call 1-800-992-4489 for details. Advanced payment by credit card or check is required. Purchase orders do not constitute payment.

Registrations must be received 10 business days before the summit start date. After this date, registrations will be taken on a space-available basis and a $25 late registration fee will be charged.

LODGING AND TRAVEL
The Supervision and Management Summit will be held at the Hard Rock Hotel, 4455 Paradise Rd, Las Vegas, NV 89109. For a rate of $79 ($149 Friday) call 800-693-7625 by January 20th and mention Falmouth Institute.

CANCELLATIONS OR SUBSTITUTIONS
Substitutions may be made at any time. All cancellations must be confirmed at least 10 business days before the start of the summit. Cancellations occurring after this date will be charged a $75 processing fee. Full tuition will be charged for any registrant who does not show up for the summit and has not confirmed their cancellation prior to the summit start date.

PHOTO USE PERMISSION
Any person who attends a Falmouth Institute conference, seminar or other program, grants explicit permission to Falmouth Institute to use and publish his or her image or likeness, without compensation, for any usual and customary business purpose of Falmouth Institute, including promotions, advertisements and marketing for Falmouth and its programs.
